Change of Address Form

Active and retired members who have recently moved should remember to forward their new contact information to M.E.B.A. Headquarters.

We require a signature accompanying address changes. Therefore, we ask that you print out the address change form, fill it out completely along with your signature and return it to Headquarters. You can drop the form in the mail, fax it to us, or if you have a scanner then you can scan the completed document with your signature and e-mail it back to us. You can use the following information to return the form:
