How It Works

Complete 30 days of covered employment in any 6 consecutive months. You and your dependents are covered.

60 days of covered employment (work, vacation, or school) within 6 months extends coverage for another 6 months.

Hit $3,500 in out-of-pocket costs in a year and the plan pays 100% of covered expenses for your family for the rest of that year. Prescription cap is $1,500.
The Bottom Line
The plan covers you at 80% for network providers. Blue Cross contracted rates reduce what hospitals and doctors can even charge. After $3,500 out of pocket, you pay nothing.
Coverage Continues
Coverage continues for 6 months following your last day of covered employment. With banked vacation days, you can extend coverage for years without working.
